Is it better to approach the luthier in regards to buying a guitar from them, or just do it all through a shop that is selling their guitars? Is there an etiquette involved whereby one should or should not do this? Obviously this doesn't include a commissioned custom guitar.

I think of all Luthier guitars as custom, its just a matter of whether you ordered one and waited or they have one available. Of course if you wanted a special combination of woods or features that would really be custom. The only advantage I could think of going through a shop is they might have one you could play before buying. The choice is yours there is no right or wrong.

also you would eliminate the middle man of their cost, by going straight to the luthier

Depends on if you know exactly what you want or not. Shops may also have what you want now vs waiting a year or two for a direct luthier order or longer if in demand. They both have their advantages.

There are no rules. Your mission is to get the best guitar you can for the money you can afford. It’s not as easy as it sounds!

A relationship with a luthier can be an interesting and rewarding experience. Ideally you should choose a luthier who consistently produces the qualities you want from your guitar. It means being able to try a few. Otherwise you have to go for reputation and any other bits of information you can pick up including online videos. Don’t ask a luthier to make wild experiments on your behalf. Go for what they do well or find another luthier. There are some nice stories on this forum about luthiers and their happy customers.

If you go through a shop you have the “try before you buy” opportunity which is a big plus. However, this way means you might reduce your access to some great luthiers who don’t sell in this way.

In my experience prices in shops are similar to what a luthier charges –the luthier takes a big hit by selling through a shop. Shops take a big cut because they are tying up their money and taking a risk that the guitar will not sell quickly. Shops also take guitars “on commission” from luthiers and (should) charge luthiers less for the selling service.

When I got a luthier guitar from max bishop, he first sent me one he had already made to try which he also wanted feedback about interns of changing any details, which I did. I had to send it back after a week or so of course, and the actual guitar I got I had to wait months for. So it would be ideal to play one first and give feedback, but you will have to have patience as well. The dealer lets you try but the only real advantage is you don't wait for the guitar.
